If you are still having cardiac specific chest pain (crushing substernal chest pain, relived with nitro) then it sounds like you are still having some sort of cardiac ischemia aka Angina, going on. (not enough oxygen getting to your heart muscle) If this is the case then I would talk to your cardiologist ASAP.

It could just be acid reflux (GERD). I used to get radiating pains just like a heart attack until I was diagnosed. Did they check?

I’m not a doc, but I did have bad chest pain once due to stressing my ribcage (by simply lying on my side, propping up my head with one arm while reading in front of the fireplace for hours). The pain eventually went away with no after effects, however, this may not be the case with you.
Exercise can often help relieve pain if it’s not too vigourous – e.g. walking, stretching, swimming.
Lastly, it’s really important to have a quality mattress since most of us spend 1/3 of our lives on it – perhaps your sleeping position is stressing your body instead of relaxing it.

Sounds like either panic attacks or coronary artery spasms

Like someone else here said, it could be Gastritis or GERD, or some serious heartburn, due to your eating habits. I went to the doctor several times, ER, a few months back, when I thought I was going to have a heart attack. I panicked and stressed and the pain radiated to my arm and fingers. I was told I have IBS and heartburn. I know I do, because I eat too fast, do not eat regular meals, such as three a day, the least. I over eat, and there is a lot of stress in my life. Now… I don’t know.. But if you feel you cannot breathe, then the pain is most likely cardiovascular and you should see a doctor whom specializes in the heart field.
My pain can also be from the way I used to lift, I did not lift correctly, and most likely, have pulled a muscle and that mucle gives me pain or bad spasms when I stree out.

I actually went to the hospital about a week ago because i had trouble breathing, chest tightness, and chest pain. The doctor performed an EKG, chest x-ray, and blood test and after all that i was diagnosed with Pneumonia. I thought I might have heart disease but i was suprised that he said i had pneumonia.
